Following their accident, the Winchesters are taken to hospital with Dean in a coma, on the verge of death. In that state, he has an out-of-body experience and wanders around the hospital until he meets Tessa, a Reaper. She tells Dean that if he doesn't accept his impending death and insists on remaining in the hospital, he'll become the kind of vengeful ghost that he frequently hunts and kills. At the same time, the doctors are having no luck in saving Dean and neither Sam or John (who emerged from the accident relatively unharmed) can do anything. Desperate, John figures out a way to contact the Yellow-Eyed Demon and makes a deal, offering the Colt in exchange for Dean's life. Yellow-Eyes is willing to accept, but he wants more: John's soul. John says yes and Dean is revived, but when he and Sam search the hospital they discover John dead and the Colt gone.
