A lot of these TVT-style Index Pages are really unrealized categories.

And done. I've started to make the necessary changes whenever I find one of these pages as I progress through the trope list.

No. That's what I'm eliminating. "Index pages" are basically TVT's hack within PMWiki to serve the purpose of MediaWiki's categories, and we want category pages to replace them. That means moving the text from the not-really-a-trope index page to the category page and setting up a redirect in the main space to send anyone looking for it there to the category.

Having articles in "categories" is no better than the other way around, however.

On the contrary, categories are automated, self-updating structures implemented as part of the wiki. TVT-style Index pages are static lists that need to be manually updated any time their contents are likely to change -- and if users are unaware that the index page even exists, they rapidly go out of date. I fail to see any way that an "index page" can be seen as even equal to a category.
